Updated Medical Forms

Hi everyone- please take the time to fill out the updated Medical Forms. You can fill them out online to save but make sure you print them out to sign. 
These forms are good for all youth events and will be good through the end of high school (or until your insurance changes) Also remember to attach a copy of your insurance card (front & back)
